If you’re selected for Audit, you’ll be asked to submit audit information relating to all your accreditations, covering the past twelve months.

Supervisory CPD
Please show your one CBT supervisory CPD activities from the last twelve months. You must submit a Supervisory Reflective Statement – we recommend you complete this shortly after the activity.
You must also upload evidence of each of your CPD activities. For webinars with no certificate and reading, we will accept the Reflective Statement only.

Supervisory Supervision Log
Accredited supervisors should maintain regular supervision for their supervisory practice – equivalent to 5% of their caseload or at least one hour per month – with a qualified CBT therapist.

Your current supervisory supervisor must submit their reference directly to us within one month of your audit submission.

Supervisee Feedback Form
Feedback from one or two supervisees must be provided as part of the audit. They must have been completed within the past 12 months.
Your supervisees should send their reports directly to us within one month of your audit submission.

Provision of Supervision Log (Supervisor Accreditation)
You must have provided a minimum of 20 hours of CBT supervision per year.
Download the form below to log your supervision during each year. You will be required to submit this when selected for an audit.
