From November 1st 2023, we will no longer accept submissions of older supervisor report templates, and you must complete the most recent version found on the links provided below. Submission of older forms will result in delays to your supervisee’s application.
Each Wellbeing Practitioner Registration has a different Supervisor Report Template. Please ensure you complete the correct template and this matches your Supervisee's application type.

The above webpages also provide tailored Supervision information for each Registration type. Supervisors should send the report directly to wellbeing.reports@babcp.com up to one month before the online application is submitted. If we do not receive a completed application for registration with payment within one calendar month of receiving a Supervisor's Report, the report will be deleted.

If you have been asked to complete a supervisor report but have concerns regarding your Supervisee’s practice, you can choose not to complete the report if you wish. If you do decide to complete the report, you can give details regarding these concerns and their personal development plan.
Supervisors cannot report concerns regarding fitness to practice anonymously as we may need to discuss this with the applicant.
If you raise concerns, or inform us of any actions the member/supervisee has taken that may be in breach of our Standards of Conduct, Performance and Ethics, this information may be treated as a complaint against the member. Although you may have been the person to alert us to these concerns, you will not be treated as the complainant, but as a witness. We may contact you for further information.
